• New items only in original manufacturer packaging are accepted.

Bidders must complete and submit this form to represent their eligibility as an Indian Economic Enterprise. This self-certification is required in accordance with 25 U.S.C. 47 and DIAR Part 1480. The enterprise must meet the IEE requirements at the time of offer, contract award, and throughout the contract term. False or misleading information submitted on this form can lead to legal penalties.

Statement of Work (SOW)

To provide an uninterruptible supply of Liquid Petroleum (LP) gas to specific locations within the Tohono O'odham Agency.

• Provide all necessary manpower, labor, equipment, materials, supplies, transportation, fuel, tools, and supervision for LP gas delivery services.
• Deliveries will be made within Tohono O'odham Agency, Sells, Arizona.
• LP gas fuel orders will be placed by the Tohono O'odham Agency, Facility Management, based on weekly LP fuel readings.

One Year.

• Deliveries must be made prior to U.S. Federal holidays.

• Contractor must conduct business between 8:00 am and 4:30 pm, Monday through Friday, except on Federal Holidays or during government-directed facility closures.

• Contractor must maintain adequate workforce for uninterrupted deliveries.

• Contractor shall furnish all equipment and services, and ensure sufficient backup equipment is available.

• Contractor must comply with all applicable State and Federal Laws, possess a current legal license for Arizona, and use competent personnel.

• All work must comply with current codes, laws, regulations, and trade standards, including safety, state, and local building codes. The most stringent code applies in case of conflict.

• Location #1: Tohono O’odham Agency, Quarters Buildings, Quonset hut, Tohono O'odham Nation Fire Bay, BIA Transportation and Facilities Shops (Bldg#49, #162, #163 and 15 BIA Quarters Houses), BIA Circle Building #49, Sells, Arizona (Tank #1 – 6,000-gallon tank).

• Location #2: Tohono Oʻodham Nation Adult Detention, TOPD Building #64, PD Road TON Adult Detention Building #64, Sells, Arizona (Tank #1 – 1,000-gallon tank).

• Monthly payments in accordance with the Prompt Payment Act.

• Contractor shall submit an invoice(s) for LP gas delivery immediately after completion of delivery.

• Invoices must include all delivery dates and any additional deliveries at the unit price per gallon.

• Payment must be submitted electronically through the federal Invoice Processing Platform (IPP).
