The Department of Veterans Affairs (VA) uses this information to evaluate an offeror's past performance and their ability to successfully execute the proposed remote prescription processing contract for VISN 10 VA Medical Facility pharmacies. Bidders should ensure their references complete and submit this questionnaire.

• Customer/Agency Identification: Information about the entity that received the services.
• Respondent Identification: Details of the person completing the questionnaire.
• Performance Information: A detailed rating scale (Exceptional to Unsatisfactory) and specific questions covering areas such as management, personnel, quality control, problem resolution, schedule adherence, responsiveness to changes, and overall performance. Respondents are asked to provide supporting information for their ratings.
• Specific Questions: Includes inquiries about cure/show cause notices and willingness to award future contracts.
• Small Business Subcontracting: Questions regarding compliance with FAR clauses related to small business subcontracting plans.

This questionnaire is critical for bidders as it directly impacts the evaluation of their past performance. Bidders must identify appropriate references (organizations they have worked with) and ensure these references understand the importance of providing thorough and timely responses. The feedback provided in these questionnaires will be a significant factor in the VA's source selection process. The deadline for submission is January 27, 2026, 1:30 pm EST.

Solicitation / RFP / RFQ

This solicitation is for a requirements contract to provide virtual/remote pharmacist services for outpatient prescription processing for VHA VISN 10 VA Pharmacies across Ohio, Indiana, and Michigan. The primary goal is to ensure continuity of prescription processing during and after the transition to the Federal Electronic Health Record (EHR) with Oracle Health, mitigating anticipated decreases in efficiency.

• Virtual/remote pharmacists to review, process, and complete pending outpatient medication or supply orders in the Federal EHR.

• Pharmacists must be licensed, proficient in English, and pass a National Criminal History Check.

• Contractor must provide qualified pharmacists, maintain a personnel roster, and ensure all personnel complete required VA training (including Oracle training) and background checks.

• Pharmacists must have compatible hardware (including PIV card reader) and stable internet bandwidth for remote access.

• Contractor must provide monthly reports of activities performed.

• Pharmacists are responsible for order verification, not dispensing.

• Specific medications like controlled substances, DOACs, and non-formulary medications are to be skipped during processing.

• Contract Type: Requirements contract with no minimum guaranteed amount.

• Maximum Contract Value: $8,500,000.00 (inclusive of base and options).

• Period of Performance: Base Year (02/16/2026 - 02/15/2027) and Option Year 1 (02/16/2027 - 02/15/2028).

• Response Date/Time: January 16, 2026, 13:30 Eastern Time.

• All questions must be emailed to the Contracting Officer.

• Award will be based on a comparative evaluation of quotes considering Administrative items, Price, Technical capability, and Past Performance.

• Past performance will be evaluated based on CPARS ratings for PSC - Q517 within the last 4 years.

• This solicitation is set-aside for Service-Disabled Veteran Owned Small Businesses (SDVOSBs).

• NAICS Code: 621399 (Offices of all other Miscellaneous Health Practitioners) with a size standard of $10.0 Million.

• FSC/PSC Code: Q517 (Medical – Pharmacy Services).

• Individual task orders will be funded as the need arises.

• The Government reserves the right to add or remove facilities and adjust quantities.

• Offerors must submit pricing for all CLINs/SLINs and acknowledge all amendments.
