Statement of Work (SOW) / Performance Work Statement (PWS)

To outline the requirements for the regulated medical waste (RMW) disposal service for various U.S. Naval facilities and the Andersen Airforce Base in Guam.

• Pickup and Disposal: Contractor will pick up RMW from U.S. Naval Hospital Guam, Apra Harbor Branch Health Clinic, Boller Veterinary Clinic, Branch Clinic Camp Blaz, and 36th Medical Group Clinic, Andersen Airforce Base.

• Pickup Frequency: Three times per week or as needed from the main staging facility (building no. 48-E2), and as instructed or needed from other specified clinics.

• Documentation: Submit signed receipt copies of shipping documents within 30 days to the Environmental Program Manager and 36th Medical Group.

• Permitting: Provide Guam Environmental Protection Agency (GEPA) Solid Waste Processing Permit.

• Compliance: Adhere to all Federal, Local, and applicable laws regarding RMW disposal ("Cradle to Grave").

• Packaging: Use non-specific bulk packaging conforming to 49 CFR 173.197(d) and RMW containers with regulated medical waste substance labels or OSHA Universal Biohazard Symbol Marking. Contractor will use government-provided containers, which must be cleaned and odor-free.

• Weighing: Utilize a government-provided digital scale for weighing RMW prior to shipping, ensuring accurate calibration.

• Invoicing: Submit monthly invoices by the fifth business day for services rendered.

• Certificates: Provide Certificates of Destruction for the duration of the contract.

• Holidays: Service will not be required on specified Federal Holidays; pickups will be rescheduled to the next workday.

• Surveillance: Periodic inspections by the Contracting Officer's Representative (COR) for adherence to cleanliness, pickup schedules, and weights.

• Customer Complaints: Complaints will be validated by the COR, potentially requiring service redone.

• Locations: U.S. Naval Hospital Guam, Apra Harbor Branch Health Clinic, Boller Veterinary Clinic, Branch Clinic Camp Blaz, and 36th Medical Group Clinic, Andersen Airforce Base, Guam.

• Security: Maintain a current listing of employees requiring access, obtain necessary passes and identification items, return identification media upon employee departure, and adhere to travel restrictions within installations. Employees are prohibited from possessing weapons, firearms, or ammunition on installations. Contractor personnel must report security threats.

• Physical Security: Responsible for safeguarding all Government property and controlled forms. Facilities, equipment, and materials must be secured at the end of each work period.

• Traffic Laws: Comply with all base traffic regulations, including seatbelt use, speed limits (15 mph), and restrictions on cell phone use while driving.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial items for Regulated Medical Waste Disposal Services. The Defense Health Agency Contracting Activity (DHACA) is seeking responses from qualified sources. This announcement serves as the only solicitation, and no separate written solicitation will be issued.

• Provision of Regulated Medical Waste Disposal services.

• Services are required at multiple locations on Guam: U.S. Naval Hospital Guam, Apra Branch Health Clinic, Boller Veterinary Clinic, Branch Clinic Camp Blaz, and 36th Medical Group Andersen AFB.

• The contract includes a base period and two option periods, each for one year.

• Contract Type: Firm-Fixed-Price (FFP).

• Period of Performance:

  • Base: August 1, 2026 – July 31, 2027
  • Option 1: August 1, 2027 – July 31, 2028
  • Option 2: August 1, 2028 – July 31, 2029

• Estimated Quantities:

  • Base: 43,900 pounds (Hospital/Clinic) + 3,900 pounds (AFB)
  • Option 1: 43,900 pounds (Hospital/Clinic) + 3,900 pounds (AFB)
  • Option 2: 43,900 pounds (Hospital/Clinic) + 3,900 pounds (AFB)

• FOB Destination.

• Quotes must be submitted electronically via email to merlinda.m.labaco.civ@health.mil or through the Procurement Integrated Enterprise Environment (PIEE) Solicitation Module.

• All questions regarding the RFQ must be submitted by email no later than 12:00 pm Pacific on April 24, 2026.

• Quotations must be received no later than 12:00 pm Pacific on May 11, 2026.

• Evaluation Factors: Technical Capability, Past Performance, and Price. Technical Capability is rated Acceptable/Unacceptable. Past Performance is more important than Price. Award will be made on a "Past Performance Price Tradeoff" basis, seeking the best value to the Government.

• This acquisition is a 100% small business set-aside.

• NAICS Code: 562211 (Waste Treatment, Disposal, and Materials Recovery Facilities), Size Standard: $47 million.

• Offerors must be registered in the System for Award Management (SAM) and have an active DUNS number.

• The solicitation incorporates numerous FAR and DFARS provisions and clauses by reference.

• Defense Biometric Identification System (DBIDS) is required for base access. Information on obtaining a pass is provided.

• Offerors must submit a technical proposal, a firm-fixed-price quote, completed provisions, and SAM registration.
