Statement of Work (SOW)

To repair and extend the service life of existing roadway pavements at the VA Southern Nevada Healthcare System (VASNHS) by addressing deterioration from normal wear, vehicular traffic, and environmental exposure. The work includes localized asphalt and concrete repairs, crack sealing, and slurry seal applications.

• Furnish all labor, materials, equipment, tools, supervision, and incidentals for roadway crack repair, asphalt removal and replacement, pavement work, slurry seal application, and pavement marking restoration.

• Contractor must provide minimal design services as VASNHS does not have an assigned Architect/Engineer (A/E) firm.

• Contractor is responsible for determining construction means and methods, field verification, layout, grading, and compliance with codes and standards.

• Government will provide site maps, photographs, and limited as-built drawings for informational purposes only.

• Develop and submit a detailed construction schedule and phasing plan.

• Clean, prepare, and seal pavement cracks.

• Saw-cut, remove, and replace damaged asphalt sections.

• Provide a site-specific grading plan prepared and sealed by a licensed Civil Engineer.

• Remove asphalt and install new concrete pavement at identified entrances.

• Clean and prepare pavement surfaces and apply slurry seal.

• Restore pavement markings, striping, symbols, and lettering.

• Maintain traffic control, pedestrian safety, and access.

• Coordinate with the City of North Las Vegas for permitting requirements.
