Statement of Work (SOW)

To acquire non-personal professional laboratory services for colorectal screening for the Elko Service Unit (ESU) at the Southern Bands Health Center.

• Provide non-personal professional laboratory services for colorectal screening using the Shield™ blood test.

• Services are for an "on needed basis" with an estimate of 20 blood vials per month.

• Deliver Shield™ blood tests and necessary collection kits to the Southern Bands Health Center.

• Deliver test results (positive or negative) to the ordering physician within a specified timeframe (approximately two weeks).

• Provide training materials and support to healthcare providers and phlebotomists at the Southern Bands Health Center on the proper use of the Shield™ screening test.

• Contractor performance and abilities will be evaluated based upon performance standards contained within this statement of work.

• Contractor must provide work consistent with the guidelines and procedures of the IHS.

• Base period: 12 months from the date of contract award.

• Option periods: Up to four (4) additional one-year option periods, contingent on satisfactory performance and government needs.

• Location: Southern Bands Health Center, 515 Shoshone Circle, Elko, NV 89801.

• The Shield™ test is a non-invasive screening option approved by the FDA for individuals aged 45 and older.

• The contract aims to increase screening rates and improve early detection of Colorectal Cancer (CRC).

• Contractor shall not attempt to bill or collect from any IHS patient or alternate resource for services provided under this award.

Sources Sought / Market Research

This document is a Sources Sought notice issued by the Indian Health Service (IHS), Phoenix Area Office on behalf of the Elko Service Unit. It is intended to identify potential contractors capable of providing 120 units of The Shield™ blood tester, an FDA-approved, non-invasive screening option for individuals at average risk for Colorectal cancer (CRC). This is for NEW equipment only; remanufactured or "gray market" items are not acceptable.

• Equipment: 120 each of The Shield™ blood tester.
• Condition: Must be NEW equipment only.
• Authorization: Vendors must be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor, or authorized reseller. A copy of the authorized distributor letter from the manufacturer is required to verify this status.
• Warranty & Service: Equipment must be covered by the manufacturer's warranty, and warranty and service must be in accordance with OEM terms and conditions.

This is NOT a Request for Proposal (RFP). It is a market research tool to assist IHS in developing a potential RFQ/RFP and determining the appropriate procurement method (small business set-aside or full and open competition).
