Scope of Work
The contractor will relocate the existing Patient Specimen Collection Restroom door from inside the Lab Draw room to the corridor. This includes closing the existing opening, installing a new door in the corridor with new hardware, and adding a specimen passthrough collection window/shelf. The work also involves framing and finishing the wall where the existing door is removed, cutting into the CMU block wall for the new opening, and installing a threshold. All work must conform to applicable codes (OSHA, NFPA, IBC, NEC, FGI, Joint Commission) and include Infection Control Risk Assessment (ICRA) and safety hazard mitigation. Submission Requirements
Proposals must include:
- A Firm, Fixed price quote on the SF18 form and a completed bid sheet.
- Technical specifications of the Statement of Work and an acceptable materials list.
- Two relevant past performances (organization name, point of contact, project summary, award amount).

Evaluation Criteria

Failure to provide the SF18 quote or technical specifications will render the entire quote unacceptable. The Government reserves the right to make an award without discussions.
