Statement of Work (SOW) / Performance Work Statement (PWS)

To relocate the existing Patient Specimen Collection Restroom door from inside the Lab Draw room to the corridor. This includes closing the existing opening, installing a new door in the corridor, and adding a specimen passthrough collection window/shelf.

• Remove the existing pass-through door from the Lab Draw room.

• Reuse and relocate the existing door in the corridor, installing it in a new jamb with new hardware.

• Frame and finish the wall where the existing door is removed, matching existing wall coverings. This includes metal framing, patching, painting, and texture.

• Install a specimen passthrough cabinet/shelf in the upper half of the new door opening.

• Cut into the existing CMU block wall for the new door opening and add new structure and framing for structural integrity.

• Install a threshold at the base of the new door opening.

• Perform an Infection Control Risk Assessment (ICRA) and implement safety hazard mitigation, including zipper walls and poly sheeting.

• Supply all equipment, parts, and labor for the relocation, wall construction, and passthrough cabinet installation.

• Workmanship must conform to the latest edition of applicable codes, including OSHA, NFPA, IBC, NEC, FGI Guidelines for Design and Construction of Health Care Facilities 2014, and Joint Commission Codes.

• Contractor is responsible for dust containment, preparation, teardown, and cleanup.

• Contractor must abide by all Safety, Security, and Infection Control Policies and Procedures of the IHS facility.

• Warranty: 1-year from the date of final acceptance.
