Statement of Work (SOW) / Performance Work Statement (PWS)

To ensure the consistent, high-quality delivery of special education and related services to students with disabilities enrolled at Santa Rosa Day School (SRDS) in Sells, Arizona. This contract is short-term to address staffing shortages.

• Provide individualized, school-based special education services in accordance with IDEA 2004, Section 504 of the Rehabilitation Act, and 25 CFR Part 39.

• Services include direct instruction, IEP development, progress monitoring, and collaboration with families and school staff.

• Key responsibilities include teaching and instruction, case management, child find activities, IEP facilitation, planning and organization, administrative and professional development, documentation and reporting, parent connection and communication, and communication with the SPED Coordinator and School Administrator.

• Ensure timely and compliant service delivery (e.g., 100% of services initiated within IDEA timelines, all IEP meetings attended).

• Improve student access to specialized support (e.g., 95% of students receive services at the frequency and duration specified in IEPs).

• Enhance student outcomes through direct services (e.g., 100% of students show measurable progress documented quarterly).

• Support culturally responsive practices (e.g., 100% of providers complete annual cultural competency training).

• Strengthen school-based teams through collaboration and professional development.

• Ensure accountability and data-driven decision making (e.g., 100% of service logs entered weekly).

• Provide responsive and ethical professional services (e.g., 100% of staff maintain active licensure).

• Period of Performance: One base year plus one option year.

• Place of Performance: SRDS campus, Sells, Arizona.

• Core Duty Hours: Monday-Friday, 7:30 AM to 4:00 PM. Direct services typically occur between 8:00 AM and 3:30 PM, with earlier release on Fridays.

• Vendor must recruit and retain qualified personnel.

• Personnel must comply with IDEA timelines and BIE protocols.

• Onboarding and training for NASIS, school procedures, and cultural competency are required.

• Service continuity must be maintained.

• Performance-based metrics tied to student progress, compliance, and service quality must be met.
