Bidders must complete and submit this certificate as part of their offer. It is crucial for determining the eligibility of offered products under the Buy American Act. Failure to properly complete this form may impact the evaluation of the offer. The government will evaluate offers based on the policies and procedures in FAR Part 25.

A search for small businesses under NAICS Code 339112 identified 988 small businesses. The "rule of 2" was not met.
• Contracting Officer Certification: The purchase is certified as accurate and complete by the Contracting Officer.

This is a Request for Quotation (RFQ) for a brand-name-only "Spyglass Discover Package" for the Iowa City VA Medical Center. The acquisition is being conducted as "Full and Open" and is under FAR 12 Commercial & FAR 13 Simplified Acquisition Procedures. The government intends to award a single, firm-fixed-price contract without discussions.

• 1 each Discover Digital Controller
• 1 each Autolith Touch EHL Generator
• 18 each Discover T/A Catheter

The evaluation approach will use a comparative evaluation process, considering Technical Acceptability (including a signed Buy American Certificate) and Price. The government may consider quotations other than the lowest price if they provide additional benefit.
