This document provides the technical specifications and pricing structure for medical supplies required under RFQ No. 19RP3826Q0061. Bidders are instructed to complete only the fields highlighted in green.

• Itemized List of Medical Supplies: Includes quantities and units for items such as tourniquets, gauze, hemostatic agents, bandages, chest seals, airways, IV kits, dressings, gloves, and splints.
• "Or Equivalent" Clause: For most items, an equivalent product may be offered, provided it meets the specified technical requirements. Bidders must submit manufacturer brochures, specifications, and/or photographs to verify compliance.
• Pricing Fields: Bidders must indicate the unit price, currency, and total amount for each item. A "Total Item Cost" field is provided.
• Shipping and Delivery: Instructions are given for shipping costs to the U.S. Embassy Manila. Offshore vendors must use Delivered Duty Paid (DDP) Incoterm. All items must be delivered between May 6-8, 2026.
• Total Costs: Fields for "Total Delivery Cost" and "Grand Total (Items + Delivery under DDP Incoterm)" are included.

Solicitation / RFP / RFQ

This document is a Request for Quotation (RFQ) for the supply and delivery of medical supplies. The U.S. Embassy Manila is seeking quotations for these items.

• Supply and delivery of various medical supplies as detailed in the attached specifications.

• All items must meet or exceed specified minimum technical requirements.

• Delivery must be made under Incoterms DDP (Delivered Duty Paid).

• Price offers shall be firm-fixed price, VAT exempt.

• The U.S. Government is exempt from Philippine ad valorem/specific tax, customs, and duties.

• Payment will be made via Electronic Fund Transfer (EFT) within 30 days of receipt of the correct statement of account.

• For actions over USD $40K, prospective vendors must be registered with the System for Award Management (SAM).

• Quotations are due by April 29, 2026, no later than 12:00 NN (Philippine local time).

• Offers must be submitted in English.

• Electronic submissions are acceptable via email to ManilaPurchasing@state.gov.

• Submission format: MS-Word, MS-Excel, or Adobe Acrobat (PDF).

• File size must not exceed 10MB per email; multiple emails are allowed for larger submissions.

• Award will be made to responsible offerors in accordance with FAR 9.104-1.

• Not a small business set-aside.

• U.S. firms are eligible to be paid in U.S. dollars.

• Foreign firms will be paid in local currency.

• The attached FAR Clauses will form part of the resultant order.

• Delivery period: 06-08 May 2026.

• Offerors must submit Standard Form (SF-18), Technical Specifications (Excel File), completed FAR/DOSAR Clauses, Technical Documentation, and Past Performance and Delivery Capability information.
