To convert the fitness area within the JWIC building at the Fort Duchesne PHS Indian Health Center into an office workspace. This includes accommodating departmental needs and installing a new furnace.

• Provision of additional furniture to meet departmental needs.
• Installation of a new furnace.

• Contractor shall provide all necessary service, labor, materials, and equipment.

• Adherence to Occupational Safety and Health Act (CFR1926) and U.S. Environmental Protection Agency and Utah State Department of Health codes and standards.

• Contractor must supply all required worksite safety equipment and comply with Fort Duchesne PHS Indian Health Center facility and safety policies.

• The Government reserves the right to inspect all aspects of work performed.

• Partial payments require an invoice and progress report submitted to the contacts listed.

• Final payment will only be authorized upon completion of all work.

• Payments are to be submitted, approved, and issued by IHSPHOENIX, ATTN: FINANCIAL MANAGEME, Phoenix, AZ.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation (RFQ) for commercial items for the Indian Health Service (IHS) – Phoenix Area Office (PAO). It constitutes the only solicitation, and a written solicitation will not be issued. The requirement is for "Systems Furniture" and is NAICS coded 337127. This is a 100% Full and Open requirement on SAM.GOV.

• Procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items.

• Vendor must be an Original Equipment Manufacturer (OEM) authorized dealer, distributor, or reseller.

• A copy of the authorized distributor letter from the manufacturer is required to verify vendor authorization.

• Items are to be shipped FOB Destination. Shipping costs must be included in the vendor quote and cannot be a separate line item.

• The Government will not split the requirement.

• Quotes must include all aspects of the RFQ, including all equipment, services, and options.

• The government intends to award a purchase order.

• The contract will be awarded to the responsive and responsible vendor providing the lowest price technically acceptable.

• Submit written quotes only; oral offers will not be accepted.

• All firms must be registered with the System for Award Management (SAM).

• Quotes must include a copy of the authorized distributor letter.

• Pricing will be evaluated in whole, including all service options and optional accessories.

• Evaluation criteria include: Price, Technical Acceptance of Product, and Delivery.

• Past performance will be evaluated using PPIRS and FAPIIS data.

• Delivery LOCATION: UO Service Unit, 6822 East 1000 South, Fort Deschesne, UT 84026.

• Invoices must be prepared monthly and submitted electronically via the Invoice Processing Platform (IPP).

• Offerors are encouraged to register with IPP if they have not already done so.

• The document incorporates numerous FAR clauses by reference and in full text, covering various aspects of commercial item acquisitions, including representations, certifications, contract terms, and conditions.

Statement of Work (SOW)

To provide and install system furniture services for the Uintah and Ouray Health Clinic, reconfiguring the existing space as shown on an attached drawing.

• Submit a proposed furniture plan for approval before ordering.

• Provide all new furniture to match existing appearance and be compatible with existing infrastructure (mechanical, electrical, phone, data).

• Reconfigure and install new furniture to create fully functional office furniture (cubicles).

• Extend phone and data connections throughout reconfigured spaces.

• All new workstations must include an electrically assisted adjustable sit-to-stand work surface.

• Provide a qualification packet including business capability, project history, licenses, and technician lists.

• Adhere to the International Building Code (latest edition) and Occupational Safety and Health Act (CFR1926).

• Comply with OSHA safety regulations and provide all required worksite safety equipment.

• All materials must be approved by the owner.

• Contractor is responsible for disposal of all project-generated debris and trash.

• Construction activities must be coordinated with clinic staff to avoid impacting daily operations and safety. Work shall occur after hours or on weekends.

• Contractor is responsible for relocating ancillary furniture as needed.

• Government retains excess existing furniture.

• Government reserves the right to inspect all work.

• Final acceptance is based on an acceptable final inspection and verification testing.

• Contractor shall provide a construction schedule.

• Facility operating hours are 8:00 AM – 5:00 PM, Monday through Friday (excluding Federal holidays).

• 30 days from the Notice to Proceed (NTP).

• Final payment contingent upon completion of all work, punch list, and final inspection.

• Contractor shall warrant all work for one year from the date of acceptance.
