To convert the fitness area within the JWIC building at the Fort Duchesne PHS Indian Health Center into an office workspace. This includes providing additional furniture and installing a new furnace.

• Provide service, labor, materials, and equipment to complete the conversion.
• Procure and install additional furniture to accommodate departmental needs.
• Install a new furnace.

JWIC building, Fort Duchesne, UT 84026 (Specific address: 6932 East 1400 South Fort Duchesne, UT 84026).

• Contractor must adhere to Occupational Safety and Health Act (CFR1926) and U.S. Environmental Protection Agency and Utah State Department of Health codes and standards.

• Contractor must supply all required worksite safety equipment and comply with Fort Duchesne PHS Indian Health Center facility policies, including the safety policy.

• The Government reserves the right to inspect all aspects of work performed and will verify and approve all material and workmanship.

• Partial payments require an invoice and progress report submitted to the contacts listed.

• Final payment will be authorized only after all work is complete, with an invoice and progress report.

• Payments will be submitted, approved, and issued by IHSPHOENIX in Phoenix, AZ.

Sources Sought Notice

This document is a Sources Sought notice issued by the Indian Health Service (IHS), Phoenix Area Office on behalf of the U&O Service Unit. The purpose is to identify contractors capable of providing system furniture for the JIWC building. This is a market research tool to assist IHS in developing a future RFQ/RFP and determining appropriate procurement procedures (e.g., small business set-aside vs. full and open competition).

• Procurement is for NEW Equipment ONLY; no remanufactured or "gray market" items.

• Vendors must be an Original Equipment Manufacturer (OEM), authorized dealer, authorized distributor, or authorized reseller.

• The proposed equipment/system must ensure OEM warranty and service are provided and maintained by the OEM.

• Quotes/responses must include a copy of the authorized distributor letter from the manufacturer.

• The government will not split this requirement.

• Place of Performance/delivery/service: U&O Service Unit, 6822 East 1000 South, Fort Duchesne, UT 84026.

• Payment will be made through IPP (Invoicing Processing Platform).

• HHSAR 352.232-71 Electronic Submission of Invoice Payment Requests (Feb 2022) will apply.

• This is NOT a Request for Proposal (RFP).

• Responses should identify socio-economic status in accordance with SBA size standards.

• Reference: Final Rule on Buy Indian Act. (SET ASIDE FOR Native-owned Small business) ISBEE.

• Subcontracting limitations are outlined in FAR 52.219-3, 52.219-14, 52.219-27, 52.219-29, and/or 52.219-30.

To provide system furniture services for the Uintah and Ouray Health Clinic, including reconfiguration of the existing space and installation of new furniture.

• Submit a proposed furniture plan for approval.

• Provide new furniture compatible with existing furniture in appearance and function.

• Reconfigure and install new furniture to create fully functional office cubicles.

• Extend phone and data connections throughout reconfigured spaces.

• Equip all new workstations with electrically assisted adjustable sit-to-stand work surfaces.

• All materials must be approved by the owner.

• Debris and trash generated by the project will be disposed of by the contractor/installer.

• Workmanship and materials are subject to final inspection and verification testing.

• Contractor shall warrant all work for one year from the date of acceptance.

• Location: 6932 East 1400 South Fort Duchene Utah 84026.

• Performance Period: 30 days from the Notice to Proceed (NTP).

• Construction activities shall occur after hours during the week or on the weekend to avoid impacting daily operations.

• Facility daytime hours of operation are 8:00 AM – 5:00 PM, Monday through Friday (excluding Federal holidays).

• Provide a qualification packet including business capability, similar projects, licenses, certifications, and qualified technicians.

• Adhere to International Building Code (latest edition) and Occupational Safety and Health Act – CFR1926.

• Comply with all required worksite safety equipment.

• Coordinate construction activities with clinic staff to minimize disruption.

• Relocate ancillary furniture as necessary.

• Provide a project schedule indicating construction dates.
