• The contract will be awarded as a purchase order.

• The government will select the quote that represents the best benefit to the Government at a price that can be determined reasonable.

• Evaluation factors include Price, Technical Acceptance of Product, and Delivery.

• Past performance will be evaluated using PPIRS and FAPIIS data.

• Submit written quotes only; oral offers will not be accepted.

• All responding firms must be registered with the System for Award Management (SAM).

Statement of Work (SOW)

To provide and install system furniture services for the Uintah and Ouray Health Clinic, reconfiguring the existing space as shown on an attached drawing.

• Submit a proposed furniture plan for approval by U&O Management before ordering.

• Provide all new furniture to match existing appearance and be compatible with existing systems (mechanical, electrical, phone, data).

• Reconfigure and install new furniture to create fully functional office cubicles.

• Extend phone and data connections throughout the reconfigured spaces.

• All new workstations must include an electrically assisted adjustable sit-to-stand work surface.

• Provide a qualification packet including business capability, project history, licenses, and technician qualifications.

• Adhere to International Building Code and Occupational Safety and Health Act (CFR1926).

• Comply with all worksite safety regulations and provide necessary safety equipment.

• All materials must be owner-approved.

• Contractor is responsible for disposal of all project-generated debris and trash.

• Construction activities must be coordinated with clinic staff to avoid impacting daily operations and safety. Work is to occur after hours (evenings/weekends).

• Excess existing furniture will be retained by the Government.

• Facility operating hours are 8:00 AM - 5:00 PM, Monday-Friday (excluding Federal holidays).

• Performance Period: 30 days from the Notice to Proceed (NTP).

• Final payment requires completion of all work, punch list, and final inspection.

• Contractor shall warrant all work for one year from the date of acceptance.

Sources Sought Notice

This document is a Sources Sought notice issued by the Indian Health Service (IHS), Phoenix Area Office on behalf of the U&O Service Unit. The purpose is to identify contractors capable of providing system furniture for the JIWC building. This is a market research tool to assist IHS in developing a future RFQ/RFP and determining appropriate procurement procedures (e.g., small business set-aside vs. full and open competition).
