This document is the "Indian Affairs Indian Economic Enterprise Representation Form." Statement of Work (SOW) / Performance Work Statement (PWS)

The Department of the Interior (DOI), Indian Affairs (IA) requires vital records for identity resolution to support fiduciary duties in heir identification, trust asset management, and probate case streamlining.

This requirement is for a Vital Records Suite, specifically the VitalChek brand name.

• Direct Access: Provisioning of the proprietary VitalChek network.

• Capacity: Support for 6,000 requests.

• Admissibility: Records must be sourced directly from government jurisdictions to ensure legal admissibility in probate court.

• User portal access with real-time reporting

• "White Glove" customer service, including manual processing and order adjudication

Solicitation / RFP / RFQ

This document is a combined synopsis/solicitation for commercial products or services, prepared in accordance with FAR Subpart 12.6. It constitutes the only solicitation, and quotes are being requested. The government is procuring "Vital Records for IA Probate Strike Team" services.

• The core requirement is detailed in Attachment 1, Statement of Work (SOW).

• The Product/Service Code is 7630, with a description of "NEWSPAPERS AND PERIODICALS".

• A quantity of 6000 Each is listed.

• The anticipated award type is a Firm-Fixed-Price (FFP) Purchase Order.

• The evaluation will be based on the Lowest-Priced, Technically Acceptable quote.

• Submission must include: Unique Entity ID, Business Mailing Address, Contact Name/Phone/Email, RFQ Number, Quote Number/Date, Pricing, and an affirmation that records will be provided by VitalChek per the SOW.

• A signed and completed IEE Representation Form must be submitted with the quote.

• This requirement is set-aside 100% for Indian Small Business Economic Enterprises (ISBEEs).

• The North American Industry Classification System (NAICS) code is 519290, with a size standard of 1,000 employees.

• The solicitation incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2025-06 (October 1, 2025).

• Points of Contact (POC) for solicitation information are listed as TBA (To Be Provided at Award).
