This document is the Offeror's Past Performance Information (PPI) form, to be completed by potential bidders. It is intended to gather information about previous contracts to help the Contracting Officer evaluate past performance.

• Contract Information: Details of current or previous contracts, including contract number, date of contract, and contract period.
• Customer Information: Name of the customer (entity that provided the goods/services), point of contact, address, and contact details (phone, fax, email).
• Contract Description and Total Amount: A description of the contract's requirements and the total amount awarded.
• Performance Rate: A section to indicate if any performance rate was provided by the customer regarding quality and timeliness, with an option to attach supporting documentation if the answer is yes.
• Other Necessary Information: A space to provide any additional information that may assist the Contracting Officer in evaluating past performance, such as letters of appreciation.

Bidders must complete this form accurately to provide details about their past performance on relevant contracts. This information is crucial for the government's evaluation of the offeror's ability to successfully perform the current opportunity. Specific instructions are provided regarding the retention period of PPI (3 years after completion) and the format for submitting multiple contracts (one form per contract, up to five contracts).

• The PPI shall not be retained for source selection information longer than three (3) years after contract completion.
• If an offeror has multiple contracts, a separate form must be completed for each, with a maximum of five forms allowed.

Performance Work Statement (PWS)

To remove approximately 300 mattresses from the YRBM-58 berthing barge at Naval Base Guam, transport them to a contractor-provided climate-controlled storage facility, store them to prevent damage, and return them to the Government upon request.

• Mattress Removal: Remove mattresses from barge berthing spaces, handling them carefully to prevent damage, tearing, or contamination. Use appropriate labor and equipment for tight shipboard spaces and ensure no damage to shipboard structures.

• Transportation: Transport mattresses to the contractor's storage facility using covered or enclosed vehicles to protect from weather and contamination. Comply with base access and escort requirements.

• Climate-Controlled Storage: Provide a secure, indoor facility maintaining temperatures between 68-75°F and relative humidity between 40-60%. Mattresses must be stored elevated, protected from pests, moisture, and direct sunlight. An inventory log must be maintained.

• Accountability & Reporting: Provide a receipt/inventory list upon pickup. Maintain a storage log documenting environmental conditions. Immediately notify the Government of any damage or issues.

• Return of Mattresses: Upon Government request, return all mattresses to Naval Base Guam within 5 business days, in the same condition as received, with labor provided for offloading.

• Mattresses handled to prevent tearing, staining, moisture exposure, or deformation.

• Transported in covered/enclosed vehicles; loads secured.

• Storage facility maintained at 68-75°F and 40-60% relative humidity.

• Stored indoors, elevated, protected from moisture, pests, sunlight.

• 100% inventory accuracy maintained.

• Environmental conditions monitored daily.

• Removal from barge completed within 3 business days of NTP.

• Return delivery completed within 5 business days of Government request.

• Any deficiencies corrected at no additional cost.

• Period of Performance: March 2, 2026 – June 20, 2026

• Place of Performance: Apra Harbor, Naval Base Guam, Guam. Berth location for YRBM-58 to be designated.

• Contractor provides all labor, equipment, vehicles, and storage space.

• Personnel must comply with local laws and Guam Naval Base security and access requirements.

• Contractor vehicles subject to inspection.

• Government reserves the right to reject mattresses exhibiting mold, mildew, moisture damage, or physical damage caused by the contractor.

Solicitation / RFP / RFQ

This solicitation is for the removal, transport, and climate-controlled storage of approximately 300 mattresses from the YRBM-58 berthing barge at Naval Base Guam. The mattresses must be stored in a manner that prevents mold, mildew, moisture damage, or physical degradation, and returned to the Government upon request.

• Removal of approximately 300 mattresses from the YRBM-58 berthing barge.

• Transportation of mattresses to a contractor-provided climate-controlled storage facility.

• Storage of mattresses in a climate-controlled facility, ensuring protection from mold, mildew, moisture damage, and physical degradation.

• Return of mattresses to the Government upon request.

• Contract Type: Firm Fixed Price.

• Period of Performance: March 2, 2026, to June 20, 2026.

• Offer Due Date: February 17, 2026, at 09:00 AM Local Time.

• Submission instructions are detailed within the solicitation, including the requirement to submit a proposal on Standard Form 1449 and Past Performance Information.

• Evaluation will be based on Price and Past Performance (Best Value Trade Off).

• This acquisition is a competitive, total small business set-aside action among sources duly authorized to operate and do business in Guam.
