Statement of Work (SOW) / Performance Work Statement (PWS)

To reseal, resurface, and restripe all applicable lines on roads and walking paths at the VA Illiana Health Care System to ensure safe travel and pedestrian crossing.

• Base Bid Tasks:
  • Fill cracks and small defects on approximately 243,647 sq ft of road surfaces with approved asphalt crack sealer.
  • Paint road markings using approved asphalt paint after sealant application. Stripe colors to be determined.
  • Resurface approximately 307,580 sq ft of parking lots by milling 2 inches and applying new asphalt, ensuring proper drainage and matching existing markings unless otherwise directed.
  • Fill cracks and small defects on approximately 34,175.56 sq ft of parking lots with approved asphalt crack sealer, matching existing stripes unless otherwise directed.
• Traffic Control: Contractor is responsible for directing traffic while road work is occurring.

210 Calendar Days after issuance of Notice to Proceed.

• Coordination: Direct coordination with VA Illiana Health Care System (VAIHCS) Facility Management Service is required prior to commencing work. Primary point of contact: Andrew Butts at (447) 792-8493.

• Records Management: Contractor must comply with all applicable records management laws and regulations, including NARA policies and the Federal Records Act. All records created for Government use are Federal records. Contractor must maintain records, prevent unauthorized destruction, and report any incidents. Records must be managed according to Federal law, and electronic records must have sufficient technical documentation. Contractor shall not remove material from Government facilities without express written permission. Records no longer required shall be returned to VA Illiana control.

• Subcontracting: Contractor must obtain Contracting Officer approval before engaging subcontractors that require disclosure of contract-generated information. Subcontractors must abide by Government and VA Illiana guidance for protecting sensitive information.

• IT Equipment: Only Government IT equipment tied to the contract and in accordance with VA policy may be used.

• Data Ownership: VA Illiana owns all data and records produced. The U.S. Government has unlimited rights to use, dispose of, or disclose such data.

• Training: Contractor employees must complete VA Illiana-provided records management training, including initial and refresher training. The contractor is responsible for confirming training completion.

• Flow Down to Subcontractors: Contractor must incorporate these requirements into all subcontracts and require written acknowledgment. Subcontractor violations are attributed to the prime contractor.

• Certification and Accreditation requirements and Security Accreditation Package are not required for this contract.
