Statement of Work (SOW) / Performance Work Statement (PWS)

To furnish all labor, parts, tools, materials, equipment, transportation, personnel, management, and supervision necessary to install two 60 Amp receptacles in Building 5045 at Davis-Monthan Air Force Base (DMAFB), AZ.

• Install two 115/230 VAC, 4-wire, 3-phase, 60-amp receptacles.

• Receptacles must be wall-mounted and compatible with Purge Unit Plug, Cat No. CPH-6034BC.

• Receptacles must be rated for Hazardous Location Class I, Group C, with delayed-action contacts.

• Remove and properly dispose of damaged or improper connectors and associated hardware.

• Ensure installation does not interfere with existing equipment or operations.

• Provide all necessary components for a complete and functional installation.

• Perform functional testing to verify proper operation upon completion.

• Submit shop drawings or specification sheets for receptacle compatibility.

• Submit a schedule within 7 days of receiving all materials/parts.

• Installation must be in accordance with manufacturer's specifications, industry standards, and business practices.

• Work areas must be kept free from waste material, rubbish, tools, equipment, and materials.

• Work areas must be left clean, neat, safe, and in a workmanlike condition upon completion.

• No asbestos-containing materials may be used or installed.

• Trash and waste materials must be removed and disposed of at a permitted site.

• Installation services to be performed between 7:30 a.m. and 3:30 p.m., Monday through Friday, excluding Federal Holidays.

• Specific scheduling requires coordination with the 355 CMS point of contact.

• Project completion expected within 7 days after receiving all materials/parts.

• Building 5045, Davis-Monthan Air Force Base, AZ.

• Contractor employees must be certified and qualified for equipment installation.

• Proof of employee certification/qualification must be provided within one business day upon request.

• All work must comply with the National Electric Code, ANSI/NFPA 70, and other applicable laws and standards.

• Employees must be fluent in English.

• Notify Contracting Officer in writing at least 21 days prior to any required utility shutdowns.

• Contractor employees must not loiter in working areas and limit travel on DMAFB to specific service areas.

• Employees must present a neat appearance and be easily recognized as contractor personnel (uniforms or name tags).

• Contractor employees must possess Company ID badges and any Government Base access passes.

• Comply with all safety regulations, including OSHA.

• Provide all necessary personal protective equipment.

• Contractor employees are prohibited from possessing weapons, firearms, or ammunition on base.

• Comply with Controlled Unclassified Information (CUI) requirements.

• Immediately report any security threats to DMAFB authority.

• Safeguard all Government property and controlled forms.

• Obtain Government identification documents for all employees.

• Contractor vehicles must be registered, meet safety/emissions requirements, and display a company logo.

• Visitors and personnel require REAL ID compliant identification for base access.
