Scope of Work
The contractor will provide all labor, materials, and supervision for the installation of automatic door closures on the Operating Room (OR) main entryway and the Prosthetics main entryway. Specific requirements include:
- Installation of one (1) OR automatic door closure (ED100 Operator - Narrow 4"x 6" Header Surface Applied Pair) and one (1) Prosthetics automatic door closure (ED100 Operator - Narrow 4"x 6" Header Surface Applied Single), both requiring wave plates.
- Use of proprietary materials and parts as per quote, ensuring compatibility with existing automatic doors.
- All personnel must be AAADM Certified Swing/Slide/Fold Automatic Pedestrian Door Inspectors.
- Work must be completed outside of standard working hours (8:00 am - 4:30 pm EST), except on Sundays and federal holidays.
- Upon completion, a written work report detailing tasks, functionality, corrective actions, parts used, and recommendations is required within five days.

The contractor is responsible for wiring the controller and wave plate for the Prosthetics door, and the VA will install the strike plate for that door. No changes to existing hardware are required.
