Bidders may need to provide this questionnaire to their past clients or subcontractors to gather past performance information. The completed questionnaire will be used by the government to evaluate the bidder's past performance on similar contracts. Statement of Work (SOW) / Performance Work Statement (PWS)

To procure a five-year, firm-fixed-price, blanket purchase agreement (BPA) for a cost-per-kit (CPK) equipment reagent rental agreement for automated special stains instruments. The goal is to enable faster and more consistent analysis of tissue samples at the James Mountain Inhofe VAMC in Tulsa, Oklahoma.

• Delivery and Installation: To the James Mountain Inhofe Veterans Affairs Medical Center (JMIVAMC), 440 S Houston Avenue, Tulsa, OK, 74127.

• Instrument Functionality: Must be a stand-alone, fully automated instrument for staining anatomic pathology fixed tissue slides. Capabilities include baking, deparaffinization, and staining of special stains.

• Capacity: Must hold 1-20 slides per carousel and have a minimum of 25 reagent positions.

• Temperature Control: Independent temperature control for each position, ranging from 37°C–75°C (98°F–167°F).

• Slide and Fluid Handling: Capable of staining slides sizes 25 mm x 75 mm and 26 mm x 76 mm. Must accommodate up to four bulk fluids in 3–6 L onboard containers.

• Compliance: Must be CSA, EMC, and CE compliant.

• Dimensions: Approximately 20in W x 25in D x 53.3in H (50.8 cm x 63 cm x 135.4 cm) due to space constraints.

• Stain Processing: Must be able to process a list of specified special stains with estimated annual slide volumes (e.g., AFB, Alcian Blue, Congo Red, Elastic, Giemsa, GMS, Gram, Iron, Mucicarmine, PAS, Reticulum II, Trichrome Blue). Pap Stain is optional.

• Contractor Responsibilities: Provide all equipment, parts, service, and maintenance for the CPK reagent/instrument rental. This includes routine shipping fees, validation/verification kits, workstations, hard drives, and associated service/replacement.

• Upgrades and Training: Required hardware/software upgrades and training must be provided at no additional expense.

• On-site Support: Contractor shall provide on-site support for validation studies, including statistical analysis and reporting.

• Installation Deadline: Equipment must be installed and operational by June 1, 2026. All implementation tasks must be completed prior to July 1, 2026.

• Maintenance: Vendors must specify maintenance requirements for operators and service engineers.

• Technical Support: 24/7 telephone support. On-site support available Monday-Friday, 8:00 a.m. – 5:00 p.m. central time, with a 24-hour response time for required on-site service. All labor, travel, and parts for repairs are at the contractor's expense.

• Operator Training: Provide training for up to two persons per analyzer annually at no cost, including tuition, materials, travel, lodging, and meals. Additional on-site training as needed.

• Period of Performance: Five-Year Blanket Purchase Agreement from 06-01-2026 to 5-30-2031.

• Place of Performance: James Mountain Inhofe Veterans Affairs Medical Center (JMIVAMC), 440 S Houston Avenue, Tulsa, OK, 74127.

• Value-Added Items: Uninterruptible Power Supply (UPS) for each system, and all ancillary equipment/furniture (workstations, monitors, etc.).

• Data Security: No hard drives or computer disks containing VA patient information will be removed from VA facilities. VA will remove or render unreadable sensitive data before equipment removal or turn-in.
