This solicitation is a Request for Quotes (RFQ) for the activation of the Mexico, MO CBOC (Community Based Outpatient Clinic) Physical Therapy department. The Department of Veterans Affairs (VA) requires various physical therapy equipment and related services.

Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for the physical therapy activation at the Mexico Community Based Outpatient Clinic (CBOC), serving the Columbia VA Medical Center.

The government requires the procurement and delivery of specific physical therapy equipment. All items must be delivered to and installed by the vendor at the Mexico CBOC. Delivery and installation are required on or after April 30, 2026.

• Freezer with cold packs: Capable of maintaining temperatures between -12°C and -6°C, holding 12 standard-sized cold packs (11” L x 14”), including 12 latex-free cold packs.

• Hi-low platform mat: Motorized height adjustment (18 ½”-29 ½”), 10-gauge steel base/frame, 5’ x 7’ upholstered top, UL Classified, hospital-grade plug, pearl gray base, Imperial blue tabletop, with a foot air switch and 10-year limited warranty.

• Kettlebells and rack: Rack with 3 offset tiers, storing six kettlebells (5-50 lbs). Includes six rubber-coated kettlebells (5, 10, 15, 20, 25, 30 lbs).

• Leg Press: 500 lb weight capacity, 5-year warranty, three-in-one machine for therapy, explosive workouts, and stretching. Must have a backrest, kickplate base, and pulley rope locks.

• Parallel Bars with tapered extensions: 10 ft option, motorized height and width adjustment, 1000 lb weight capacity, steel frame, 10-year limited warranty. Includes a non-skid black mat.

• Recumbent bike: 400 lbs weight capacity, self-powered, auto-start, with various workout programs and display metrics. Includes support handlebars, deluxe back support, and limited warranties on electrical, mechanical, and labor.

• Treadmill: Bi-directional belt motion, decline to -3% grade, meets UL requirements, full-length medical side rails, 5-year parts/3-year labor warranty, 500 lbs capacity, requires a dedicated 120V, 15 amp circuit.

• Hand evaluation kit: Includes a Hydraulic Hand Dynamometer for precise strength assessment, adjustable to five grip positions (1-3/8” to 3 3/8”), and comes with a warranty, carrying case, and calibration certificate.
