Offerors must answer "Yes" or "No" to each question. The form also requires offeror information, including company name and primary Point-Of-Contact details.

Completion of this questionnaire is mandatory. A "no" response to any item disqualifies the organization from receiving an Alliant 3 GWAC Master Contract award. The responses will be used by GSA for a separate risk assessment, and approval is required for award eligibility. Bidders must ensure they can truthfully answer "Yes" to all questions to be considered.

• Total Price: Calculated automatically based on proposed rates and a standard 2000 hours per labor category.

• Evidence Submission: Offerors must insert rate tables from supporting contracts/task orders into the PDF, separated by title pages that identify the Contract/Task Order Number, Labor ID(s), and Work-Site.
• Signature: Requires signature of an Approving Official, including their printed name, title, or position.
