Submission Requirements
Interested small businesses must respond via the provided survey form (https://forms.osi.apps.mil/r/tdQWYLfTXU) and include:
- Verification of small business status.
- Unique Entity Identifier (UEI) and CAGE Code.
- Documentation from bonding company showing current single and aggregate performance/payment bond limits.
- Description of Experience: Up to three projects (prime contractor or affiliate for SBs) completed within the last 5 years or >95% complete, similar in scope (HVAC upgrades, chiller plants, DDC controls) and size (10,000 SF+). Projects must demonstrate self-performing at least 15% of construction.
- For each project: percentage complete, completion date, size, scope, dollar value, self-performed portion/percentage, design-build/bid-build, and number of subcontractors.
- Confirmation of ability to self-perform at least 15% of the cost of the contract (excluding materials) with own employees, per FAR 52.219-14.
