Performance Work Statement (PWS)

• Equipment: Contractor must provide all necessary equipment, including vehicles capable of mechanically lifting and emptying dumpsters, and a specified number of dumpsters and 60-gallon receptacles. Government will furnish 32-gallon metal waste cans.

• Submittals: Required submittals include Safety Data Sheets (SDS), Abbreviated Accident Prevention Plan (APP), list of personnel, proof of insurance, equipment list, and monthly invoices/time sheets.

• Payment: Monthly payment after inspection, acceptance, and invoice processing. Invoices must be proper and submitted by the fifth of the month for the previous month's services.
