Key Requirements & Certifications
The team lead performing the survey must hold Washington State certifications in cross-connection control and backflow prevention; certifications from USC FCCCHR are not accepted. The survey must comply with federal, state, and local regulations, including DAFMAN 32-1067, AFI 48-144, UFCs, WAC Title 246-290-490, Uniform Plumbing Code, AWWA M14 Manual, USC Manual of Cross-Connection Control, and Washington State DoH Guidance. Deliverables include a Preliminary Survey Report (within 45 days of completion) and a Final Report (within 90 days of completion). Offerors must be registered in SAM.
