Amendment / Modification

• Offerors may change their submitted offer by letter or electronic communication, provided it references the solicitation and amendment numbers and is received prior to the opening hour and date.

• Unit of Measure (Q1, Q20): Delivery/setup CLINs use "EA" as the unit of measure, generally representing a single delivery event. Pricing is based on service zones, not mileage within a zone. Separate mileage charges apply only for deliveries outside defined service zones.

• Bundled Services (Q3): Delivery/setup CLINs are fully bundled and include all labor and activities described in the PWS (scheduling, setup, patient education, etc.).

• Emergency Services Billing (Q4): If an emergency order is not completed within the required timeframe due to no fault of the Government, it may be reclassified and paid as a routine delivery service if completed within two business days.

• Quantities (Q5): Price schedule quantities are based on historical usage for evaluation purposes only and are not guaranteed.

• Minimum Guarantee (Q6): The solicitation includes a guaranteed minimum ordering amount of $100,000.00.

• Pricing Structure (Q10, Q11): Offerors must submit pricing strictly in the required CLIN format. Alternative pricing models (e.g., PMPM, tiered) will not be considered.

• Facility Location (Q12): Contractors must maintain a facility within the geographic distance requirement stated in the PWS. Acceptable documentation (e.g., lease agreement) will be required.

• Loaner Equipment (Q13): Loaner equipment may be contractor-owned with prior VA approval, provided it meets functional requirements.

• Repair Authorization (Q14): Prior authorization (e.g., VA staff email, purchase order) is required before performing billable repair services.

• Inventory Tracking (Q15): A specific inventory tracking system is not mandated; contractor-developed systems are acceptable if they capture required data.

• Award Basis (Q16): Award will be made using Comparative Evaluation Procedures (FAR Part 13). Price realism is not evaluated.

• Common Deficiencies (Q19): Frequent issues include incomplete pricing, failure to acknowledge amendments, misunderstanding pricing structure, and insufficient documentation.

• VA Facility Pickup/Delivery (Q21): No charge for pickup/delivery to VA Medical Centers or CBOCs. If VA-owned equipment is picked up from a VA facility during a delivery to a Veteran's residence, no separate charge is authorized for the VA facility pickup.

• Complex Power Chairs (Q22): CLIN 0003 pricing excludes mileage. Repairs are priced using flat rates. Duplicate mileage billing across CLINs is not authorized.

• Deliveries Beyond Service Area (Q23): Separate mileage charges may apply for deliveries outside the PWS-listed areas, requiring prior approval via purchase order, VA PKI email, or verbal authorization for emergent situations.

• Procedure for segregating and handling recovered equipment.

Statement of Work (SOW) / Performance Work Statement (PWS)

This Performance Work Statement (PWS) outlines the requirements for Durable/Home Medical Equipment/Accessories (DME/HME) Services for the Veterans Health Care System of the Ozark (VHSO). The goal is to provide necessary labor, facilities, and management for VA-owned equipment, including delivery, set-up, disassembly, storage, repairs, and inventory management.

• Provide delivery and pick-up, receiving, storage, set-up, assembly, take-down, disassembly, installation, sanitizing, repair, and maintenance of VA-owned DME/HME for eligible Veterans and caregivers.

• Services include transportation, safety inspections, and non-clinical in-service training for Veterans on equipment functionality.

• Furnish DME/HME and accessories to eligible Veterans within the VHSO's primary service area, including their homes or other designated locations.

• Equipment must be provided based on a Veteran's assessment and a VAMC healthcare provider's order.

• Contractor must meet Joint Commission Standards or an equivalent body.

• Services must be performed at or above the standards set by the Joint Commission.

• Notification required for any accreditation surveys, revocations, or citations.

• Contract period: One year from the date of award, pending availability of funds.

• Place of performance: Multiple locations including Veteran/caregiver residences, VA Medical Centers/CBOCs, and Contractor or subcontractor facilities.

• Contractor must have a facility physically located within 5-10 miles of the VHSO.

• Contractor must not be on the Health and Human Services Office of Inspector General's (HSS/OIG) List of Excluded Individuals/Entities.

• Services must be provided during work hours (Monday-Friday, 8:00 a.m. to 4:30 p.m.) and potentially non-work hours or Federal Holidays in case of emergency.

• A 24/7 communication system is required for emergency services, with a four-hour response time for emergency repairs or deliveries.

• Contractor must provide a toll-free number for Veterans/Caregivers and Medical Centers/CBOCs.

• Equipment setup and assembly are required, with limitations on permanent fixtures.

• Contractor is responsible for site preparation, unpacking, assembly, and removal of packing materials in the Veteran's home.

• Re-certification and/or re-education on equipment mechanics/functionality may be required.

• Detailed record-keeping for each Veteran is required.

• Maintenance, repair, and refurbishing of DME/HME must be performed by qualified personnel.

• Contractor must provide Veteran/Caregiver education on equipment use, safety, and care.

• Specific procedures for environmental assessment, electrical safety testing, and infection control must be followed.

• Reporting requirements include Quality Control, Veteran Satisfaction Surveys, inventory reports, and incident reports.

• Contractor must comply with all applicable Federal, State, and Local laws, including fire and life safety codes.

• To personally move equipment and household items when relocating.
