This is a Sources Sought Notice for Contracted Residential Services (CRS) - Medical Respite (MR) Care Services for the Edward Hines, Jr. VA Hospital. The Great Lakes Acquisition Center (GLAC) is seeking contractors to provide a safe, clean, comfortable, sober, private, and secure environment for homeless Veterans requiring short-term recuperative care. The goal is to aid Veterans in recovery from minor ailments or injuries, prevent worsening symptoms, and reduce rehospitalization, ultimately facilitating discharge to permanent housing. Services are for VHA healthcare-eligible Veterans preparing for or recovering from medical procedures.

• Provide a low-barrier therapeutic and rehabilitative environment (not for detoxification or acute treatment).

• Facility must be within the Hines VA catchment area.

• Facility must meet all local, State, and Federal licensing, health, and fire safety standards (NFPA 101 Life Safety Code).

• At least 5-7% of beds must meet ADA accessibility guidelines.

• Ensure structural soundness, adequate ventilation, safe water supply, and proper illumination.

• Implement preventative measures against infectious diseases.

• Provide on-site private office space for Hines HCHV CRS Coordinator.

• Maintain functional Automated External Defibrillators (AEDs) and staff trained to operate them.

• Implement 24/7 security measures (staff or system, secured entry, cameras, exterior lighting).

• Provide separate accommodations for male and female Veterans, with secure segregation for females.

• Ensure food is prepared, served, and stored under sanitary conditions.

• Provide sufficient staff, including nurses (35+ hrs/week) and case managers (35+ hrs/week).

• Offer safe housing with laundry, kitchen, activity room, and restroom facilities.

• Facilitate same-day placements.

• Provide three meals daily with snacks, accommodating dietary restrictions.

• Link Veterans to permanent and transitional housing.

• Assist with Activities of Daily Living (ADL).

• Provide Harm Reduction Intervention and Housing First approach.

• Offer medication management and education (24/7 staff availability).

• Provide mental health services for assessment, intervention, and referrals.

• Contract Type: Firm, fixed-price Indefinite Delivery / Indefinite Quantity (IDIQ).

• Period of Performance: Anticipated April 1, 2026, through May 31, 2031 (five years).
