The Indian Health Service, Kayenta Service Unit is issuing this sources-sought synopsis as a means of conducting market research (10.001) in identifying prospective sources to fill a government requirement for responsible contractor to conduct a full house renovation. The project includes:
a. Replacements of front and back door and replace best lock.
b. Replacement of flooring with luxury vinyl tile.
c. Replacement of all cabinets in kitchen, restrooms, and utility room.
d. Replacement of all sinks and faucets in restrooms and kitchen.
e. Replacement of all counters.
f. Replacement of furnace with HVAC and install outside condenser and install cement base.
g. Replacement of bathtub with standing shower.
h. Replace crawl space HVAC flexible duct.
i. Replace baseboards.
j. Replace stove vent and duct system.
k. Replace all water plumbing.
l. Remove large tub at one location
m. Replace utility room subflooring with ¾ inch plywood.
n. Install one more layer of cinder block around modular home and replace vent panels.
o. Contractor is to dispose of debris at approved EPA disposal location.
p. KHC to run 220-volt power to AC condenser.
The applicable North American Industry Classification System (NAICS) code assigned to this requirement is 236118 – Residential Remodelers with a small business size standard of $45.0.

Submission Instructions: Interested parties who consider themselves qualified to perform the above-listed services are invited to submit a response to this notice by 01/15/2025 @ 12:00 PM MST to Daisha Richards at Daisha.Richards@ihs.gov with the subject line “SSN – KHC IHS1521322. Your submission must include the following information, at a minimum:
- Company Profile that includes: UEI, Name and Address, Primary POC information, business size and type of ownership, socio-economic status/participation such as 8(a), WOSB/EDWOSB, SDVOSB and HUBZone, identification of Native American ownership (if applicable), SAM.gov representations and certifications, and other related documentation.
- A brief statement of the company’s capability that relates to the described government requirement. Additional demonstration of capability may include current and past performance history for the same or similar requirement, established contracts such as GSA or VA Schedule, etc.
- If claiming Indian-owned Small Business Economic Enterprise (ISBEE) or Indian-owned Economic Enterprise (IEE), submit completed IHS IEE Representation Form (attached).
