Statement of Work (SOW) / Performance Work Statement (PWS)

To provide massage therapy services to authorized patrons of the Langley Sports and Fitness Center at Joint Base Langley-Eustis (JBLE), Langley Air Force Base, VA.

• Furnish all management, transportation, tools, supplies, equipment, and labor for massage therapy services.

• Provide massage tables, chairs, standard intake forms, service-ready linens, a printer for administrative documentation, and cleaning/sanitation supplies.

• Ensure continuous and reliable service to patrons.

• Massage Therapists must be CPR and First Aid Certified, hold current certifications for the services provided, and possess liability insurance.

• Therapists must arrive 15 minutes prior to scheduled appointments.

• Greet each customer within 5 minutes and secure payment receipt.

• Maintain an operational online calendar and utilize their own scheduling system.

• Deliverables include certificates, insurance liability, schedule of appointments, and personnel information for badges, with specified due dates.

• Performance will be evaluated based on objectives such as timely arrival, customer greeting, calendar operability, and scheduling system maintenance, with defined rating criteria for performance levels.

• Services are to be performed at the Langley Sports and Fitness Center, Langley Air Force Base, VA.

• No weapons, firearms, or ammunition are permitted on the installation.

• Smoking is restricted to designated areas.

• The contractor shall provide a contract manager and an alternate.

• The NAFI (Non-Appropriated Fund Instrumentality) will pay an 80/20 split of the monthly proceeds from the concessionaire contract. Payments are made after submission of sign-in sheets and invoices for validation and certification of satisfactory services.
