Past performance is of considerable importance to this acquisition. This questionnaire is sent to references to assess the contractor's suitability. Bidders should proactively communicate with their references to ensure they understand the importance of this feedback and can complete the questionnaire accurately and by the deadline. This solicitation is for "Kislak Painting Frames fabrication services" for the Library of Congress. The Library is opening the Kislak Gallery of the Early Americas exhibition in Fall 2026 and requires custom, conservation-grade frames, acrylic glazing, and support components for paintings on canvas. The contractor will also fit the paintings into the frames on-site at the Library of Congress prior to display.

• Fabricate and deliver eight (8) custom, conservation-grade frames for eight (8) paintings.

• Provide and install 6mm thick Optium Museum Acrylic glazing (or equivalent).

• Fit paintings into frames on-site at the Library of Congress.

• Provide and attach appropriate hanging and security hardware.

• Provide material samples for aesthetic and conservation review.

• Deliver detailed drawings of frame dimensions and assembly.

• Produce a project plan and schedule.

• Ensure all components have at least 30 days to off-gas before delivery.
