Amendment / Modification

This amendment (P00004) makes the following changes to Request for Quotation No. 140A2326Q0125:

• Estimated Hours: The estimated hours for the Base Year, Option Year 1, and Option Year 2 have been revised. The original solicitation estimated 180 days in a school year (1440 total hours) plus 336 hours for summer, totaling 1,808 hours. The correction states an estimate of 8 hours/day x 1 day/week x 37 weeks = 296 labor hours for the Base Year, Option Year 1, and Option Year 2 combined. Bidders are instructed to submit quotes for these 296 labor hours and include the overall total.
• Contract Type: The contract type has been changed from "Time and Material (including Labor Hour)" to "Labor Hour".
• Time Frames and Deliverables: The section "Page 4 of 34. TIME FRAMES AND DELIVERABLES" has been deleted. The background investigation clause, previously located here, is now covered under "No. 3. Custom Clause" on Page 12 of 34.
• Evaluation Criteria (Key Personnel): The evaluation criteria for "Key personnel" under Section 52.212-2 (Commercial Product and Commercial Services) has been clarified. Contractors must provide key personnel resumes demonstrating experience as a special education teacher, and minimum, contractors shall provide key personnel's resume demonstrating their experience to READ: as a PT/PTA.

Bidders must adjust their quotes to reflect the revised estimated labor hours (296 total) and the updated contract type (Labor Hour). The evaluation criteria for key personnel has also been clarified.

Solicitation / RFP / RFQ

This is a combined synopsis/solicitation for commercial products and services, specifically for in-person Physical Therapist (PT) and Certified Physical Therapist Assistant (CPTA) services. The Bureau of Indian Education (BIE) anticipates awarding a "Time and Material (including Labor Hour)" type purchase order.

• Provide on-site Physical Therapist and Certified Physical Therapist Assistant services at T'iis Ts'ozi Bi' Olta (Crownpoint Community School).

• Services include furnishing all labor, materials, equipment, and supervision.

• PT/PTA will evaluate and interpret evaluation results for the IEP committee, design and implement interventions, and provide teacher consultation services.

• PT/PTA will provide daily performance data, assist with goal writing, participate in IEP meetings, maintain student rosters, and submit progress reports.

• Conduct comprehensive occupational and physical therapy evaluations.

• Use standardized and norm-referenced assessment tools.

• Review and analyze current evaluation reports and student medical/educational records.

• Participate in Multidisciplinary Evaluation Team meetings and attend IEP/MET meetings.

• Provide quarterly progress notes and therapy logs.

• Contractors must provide their own equipment (laptops, etc.) and supplies.
