Performance Work Statement (PWS)

This PWS outlines the requirements for providing comprehensive housekeeping services at the Veteran Affairs George E. Wahlen Medical Center Campus (VASLC), including the main hospital (Buildings 1 and 14) and several other specified buildings.

The contractor shall provide all labor, supervision, management, transportation, equipment, materials, and non-personal services for hospital housekeeping, general housekeeping, and Regulated Medical Waste (RMW) collection. • Contractor must implement an effective quality control program and will be subject to government quality assurance surveillance.

• Performance objectives include defect limits per service type, with remedies like re-performance within 2 hours.

• Place of Performance: Veteran Affairs George E. Wahlen Medical Center Campus, 500 S. Foothill Blvd., Salt Lake City, UT 84148, covering multiple buildings.

• Hours of Operation: Services are required 7 days a week, with specific crew schedules for weekends and weekdays, including multiple shifts.

• Staffing: Key personnel (Supervisor, Shift Leaders) require specific experience and certifications. All personnel must be able to read, write, speak, and understand English.

• Training: Mandatory in-service training, annual refresher training, and initial orientation covering infection control, safety, hazardous materials, and specific hospital procedures.

• Uniforms & PPE: Contractor-provided uniforms, with specific requirements for surgical scrubs and Personal Protective Equipment (PPE) as needed.

• Health Requirements: Pre-employment physicals, immunizations (TB, Hepatitis B, Influenza, etc.), and reporting of exposures or illnesses are required.

• Security: All contractor employees are subject to background investigations and must comply with VA facility security regulations.

• Government-Furnished Property: Includes trash liners, RMW supplies, hand hygiene products, cleaning/disinfectant wipes, paper products, EPA-registered disinfectants, face masks, local phone service, and one computer for the supervisor. The government will NOT provide general equipment.

• Contractor Requirements: Contractor must furnish all other necessary equipment, supplies, and vehicles. Approval from COR is required for new supplies/equipment. Use of aerosol sprays is prohibited in patient treatment areas. Safety Data Sheets (SDS) for all chemicals must be provided.
