Statement of Work (SOW) / Performance Work Statement (PWS)

To provide a clean workplace free of snow and ice.

• Provide snow and ice removal services on all designated locations as specified on the site map.

• Services include snow plowing, sanding, salting, and ice removal.

• Work is to be performed on locations including parking lots, entrances, sidewalks, landings, and entrance ramps.

• Ice removal is to be performed immediately upon notification to provide secure footing.

• Sand and/or salt mixture usage must comply with local regulations and COR/RFOS approval.

• Services must be performed to contract standards and all federal, state, and local regulations.

• Contractor must maintain effective quality control.

• Government will evaluate performance based on a Quality Assurance Surveillance Plan (QASP).

• Performance Requirements Summary (Exhibit 1) requires 98% with no more than one valid customer complaint per month for snow/ice removal and equipment maintenance.

• Invoices will not be approved without submitted pictures verifying services performed.

• Period of Performance: 12 months from the effective date of award, with four 12-month option periods.

• Contractor shall NOT perform services unless notified and instructed by the COR/RFOS.

• Services must commence within TWO (2) HOURS OF NOTIFICATION.

• Contractor must maintain contact with COR/RFOS during operations and notify upon completion.

• Contractor must provide an on-site qualified supervisor during all snow plowing, sanding, and ice removal operations.

• Contractor personnel must communicate in English.

• Contractor must provide a list of employees and contact numbers to the COR/RFOS.

• Employees must be capable, experienced, and trained; maintain neat appearance.

• Contractor must provide an onsite representative with authority to act on behalf of the contractor.

• Contractor employees must identify themselves and comply with security requirements, including background checks and AT Level I and OPSEC training.

• Contractor is responsible for all damages to government property, vehicles, or persons caused by their operations.

• Contractor must submit Material Safety Data Sheets for hazardous materials within 10 days of award.

• Hazardous waste releases are the sole responsibility of the contractor for cleanup.

• Contractor must comply with all applicable laws, regulations, and ordinances.

• Contractor must provide State and Federal licenses, operating permits, and certifications within five working days of award.

• Monthly invoices must be submitted within five days after services are performed, with required pictures for approval.

• A monthly inspection checklist must be completed and submitted with the monthly invoice for payment approval.
