• Option 1 (12 Months): Mirrors the CLINs and estimated quantities of the base period for preventive maintenance and repair parts/labor.
• Option 2 (12 Months): Mirrors the CLINs and estimated quantities of the base period for preventive maintenance and repair parts/labor.
• Totals: The Base Year Total and each Option Year Total are estimated at $50,000.00. The Total for All Options is $150,000.00.

Statement of Work (SOW) / Performance Work Statement (PWS)

To provide Preventive Maintenance (PM) and Corrective Maintenance (CM) services for Material Handling Equipment (MHE) at Defense Logistics Agency (DLA) Disposition Services (DS) locations in Sierra and Tucson, ensuring MHE is consistently operational to support daily operations.

• Perform annual Preventive Maintenance (PM) on MHE as per Original Equipment Manufacturer (OEM) manuals and COR requests. This includes inspections, fluid checks, lubrication, filter replacement, and fork inspections.

• Perform Corrective Maintenance (CM) for MHE deficiencies, including assessment, troubleshooting, and repair.

• Provide all necessary personnel, equipment, tools, test equipment, materials, supervision, and quality control.

• Utilize lock-out/tag-out procedures for all maintenance.

• Maintain electric-powered equipment, batteries, and charging stations.

• Update Attachment 2 if equipment changes.

• PM services to be completed within ten (10) business days of start, unless rescheduled.

• CM assessments within five (5) business days of notification.

• CM repairs to be completed within five (5) business days of parts receipt, unless extended.

• Provide weekly status updates for CM actions.

• Maintain a Quality Control Plan (QCP) and submit to COR within ten (10) business days of award.

• Contract period: One (1) 12-month base period and two (2) 12-month option periods (totaling up to three years).

• Place of Performance: DLA DS Sierra and Tucson facilities. Services to be performed on USG premises to the maximum extent possible during normal business hours.

• Contractor must provide contact POC and phone number.

• Contractor employees must possess Certified Forklift Technician (CFT) certifications.

• Contractor is responsible for all spills and leaks, including cleanup at no cost to the USG.

• Contractor must comply with base access, security, and local installation policies.

• Contractor must provide all necessary Contractor Furnished Property, Materials, Equipment, and Services (CFP/M/E/S), including PPE and OEM-recommended replacement parts.

• Invoicing to be submitted monthly through DoD Wide Area Workflow (WAWF).

• Contract minimum: $2,500. Maximum: 150% of estimated contract value.
