Scope of Work
The contractor will provide on-site medical services for students at the Weber Basin JCC, located at 7400 South Cornia Dr, Ogden, UT 84405. Services are to be delivered by a Physician, Nurse Practitioner, or Physician Assistant. Key responsibilities include:
- Examinations and Treatment: Initial physical examinations, assessments, treatment of minor/acute conditions, and management of chronic conditions.
- Health Management: Prescription issuance (including Schedule II medications), immunizations, health condition monitoring, and hospitalization recommendations.
- Prevention & Education: Follow-up care, prevention planning, early identification and diagnosis, health promotion, and family planning education.
- Coordination: Collaboration with substance abuse counselors and other health/wellness components on Center, participation in committees (DAC, applicant review).
- Documentation & Reporting: Maintaining medical equipment, recording medical documentation, and submitting monthly health-related reports to the Center Director and Health and Wellness Director.
Performance Standards
Services must adhere to Federal/state/local laws, OSHA, HIPAA, AMA guidelines, and Job Corps standards. Performance will be monitored by Job Corps assessment teams. Contractor personnel must coordinate with Center staff, maintain timely documentation, and participate in required calls/training. A positive and productive manner is expected to ensure student satisfaction.

Special Requirements
