Statement of Work (SOW) / Performance Work Statement (PWS)

To provide on-site medical services for students at the Weber Basin Job Corps Civilian Conservation Center (JCCC), ensuring comprehensive healthcare that supports their academic, vocational, and life-learning goals.

• Provision of on-site medical services by a Physician, Nurse Practitioner, or Physician Assistant.

• Services include examinations, assessments, treatment, prescription issuance, immunizations, health condition monitoring, hospitalization recommendations, follow-up care, and prevention planning.

• Emphasis on early identification and diagnosis of medical problems, basic health care, health promotion, prevention, and education to help students overcome employability barriers.

• Coordination with other health and wellness components on Center.

• Analysis and use of efficient, cost-effective, evidence-based healthcare practices.

• Specific tasks include initial physical examinations, developing medical treatment plans, recording medical documentation, treating minor and acute conditions, managing chronic conditions, providing family planning education, prescribing medications (including Schedule II), collaborating with substance abuse counselors, participating in committees (DAC, applicant review), maintaining medical equipment, and reporting health-related matters monthly.

• Ensuring patient privacy and confidentiality in accordance with HIPAA.

• Participating in quality improvement activities and advising on health education.

• Coordinating with other departments on Center for integrated promotion and education services.

• Adherence to applicable Federal laws/regulations, OSHA, HIPAA, state and local laws, and American Medical Association guidelines.

• Compliance with standards set by the Office of the Director, Job Corps, US Department of Labor (DOL), and the Contractor.

• Medical services subject to monitoring and review by Job Corps assessment teams.

• Contractor personnel must coordinate with the Health and Wellness Director and/or Center Director.

• Monthly reporting on health-related matters to the Center Director and HWD.

• Timely recording of all medical documentation.

• Maintenance of medical equipment.

• Participation in monthly conference calls, webinars, and DOL training.

• Positive and productive manner to facilitate student willingness to pursue treatment plans and achieve positive student satisfaction survey results.

• Medical services required on center (Weber Basin Job Corps Center, Ogden, UT) on a weekly basis.

• Days and hours to be established in coordination with Center personnel, typically one to two days a week, Monday-Friday, 8 am – 5 pm.

• Schedule may vary based on Center OBS and PRH changes, with modifications executed via contract modification.

• Contractor personnel must possess an active, unrestricted license to practice independently in the state where services are provided.

• Maintain licensing, certification, and required insurance coverage.

• Agree to a criminal background check and drug testing if determined necessary.

• Contractor is responsible for maintaining their own malpractice/professional liability and general liability insurance.

• Adherence to Job Corps Federal Regulations (20 CFR Part 684), Technical Assistance Guides (TAGs), CMHC Desk Reference, and Job Corps Health & Wellness information.

• Contractor personnel shall not engage in research projects or publications without prior written approval.

• Security procedures of the Center must be followed.

• Student records and reports are property of the US Government.

• Invoices submitted monthly via the INVOICING PROCESSING PLATFORM (IPP).

• Past performance evaluations will be conducted annually and at contract completion via the Contractor Performance Assessment Reporting System (CPARS).
