Special Requirements & Notes

Contractor personnel must possess an active, unrestricted license to practice independently in the state of Utah, along with prescribing authority within the state. There is no reciprocity for out-of-state licenses. Personnel must agree to criminal background checks and drug testing if required, and maintain their own malpractice/professional liability and general liability insurance. Services are required weekly, typically one to two days, Monday-Friday, 8 am – 5 pm, at the Weber Basin Job Corps Center in Ogden, UT. Adherence to Federal laws/regulations, OSHA, HIPAA, state and local laws, American Medical Association guidelines, and Job Corps Federal Regulations is mandatory. Monthly reporting and participation in quality improvement activities are also required.
