This document is a Past Performance Questionnaire (PPQ) intended to gather feedback from clients regarding a contractor's performance on previous projects. The completed questionnaire will be incorporated into a proposal submission and counts towards the page limitation.

Bidders must provide this questionnaire to their clients (evaluators) to complete. The completed PPQ is a required part of the proposal submission and will be used by the government (VHA-PCAC) to assess the offeror's past performance. Bidders need to ensure timely completion and submission by their clients, as late submissions will not be accepted or evaluated.

Solicitation / RFP / RFQ

This solicitation is for the modernization of elevator components and controls at the VA Central Iowa Health Care System (VACIHCS) in Des Moines, Iowa. The project aims to ensure elevators meet current safety and operational codes and directives. The modernization includes new controls, motors, electrical systems, emergency phones, plumbing, HVAC, lighting, cables, data, fire alarms, fire suppression, and new car interiors.

• Modernization of eight elevators (A-East Passenger, A-West Passenger, B-Passenger, B-Service, C-Passenger, D-Passenger, E-Passenger, and K-Freight).

• Installation of new components including controls, motors, electrical systems, emergency phones, plumbing, air conditioning, lighting, cables, data, fire alarms, fire suppression, and new car interiors.

• All work to be performed in accordance with design information, specifications, and documents provided by the VA.

• Contract Type: Single firm-fixed-price contract.

• Period of Performance: Approximately 365 calendar days from the issuance of the notice to proceed.

• Magnitude of Construction: Estimated between $5,000,000 and $10,000,000.

• Solicitation Method: Request for Proposals (RFP) in accordance with FAR part 15.

• Evaluation: Tradeoff procedures as listed in the solicitation.

• Response Date: February 12, 2026, 2:00 PM Eastern Time.
