Eligibility & Requirements
This is a Total Small Business Set-Aside. Providers must possess an active, unrestricted license to practice independently in Washington State. Malpractice/professional liability insurance of at least $1 million and general liability insurance are required. Personnel must agree to criminal background checks and drug testing. Experience with financially disadvantaged youth is preferred. No collaborative agreement is required for Nurse Practitioners in Washington State. There is no preference between a physician or NP/PA, provided they meet licensure and scope of practice requirements.
Submission & Evaluation
Quotes must include a completed Quoter's Questionnaire, a Schedule of Items with pricing, proof of licenses and insurance, resumes for key personnel, and three professional references. Quotes lacking required information may be deemed non-responsive. Award will be made to the offeror representing the best value to the Government, based on Technical Capability, Past Performance, and Price. Questions were due by March 11, 2026.
Key Clarifications
