Statement of Work (SOW) / Performance Work Statement (PWS)

To provide on-site medical services for students at the Columbia Basin Job Corps Civilian Conservation Center (JCCC), focusing on early identification, basic healthcare, health promotion, prevention, and education to help students overcome barriers to employability.

• Provision of medical services by a Physician and/or Nurse Practitioner/Physician Assistant.

• Services include examinations, assessments, treatment, prescription issuance, immunizations, health monitoring, hospitalization recommendations, follow-up care, and prevention planning.

• Emphasis on efficient, cost-effective, evidence-based healthcare.

• Coordination with other health and wellness components on Center.

• Early identification and diagnosis of medical problems.

• Basic medical care, health promotion, prevention, and education aligned with Job Corps Policy and Requirements Handbook (PRH) Chapter 2, Section 2.3.

• Specific tasks include initial physical examinations, developing medical treatment plans, recording medical documentation, treating minor and acute conditions, managing chronic conditions, providing family planning education, prescribing medications (including Schedule II), collaborating with substance abuse counselors, participating in committee meetings (DAC, applicant review), maintaining medical equipment, monthly reporting, establishing referral systems, attending training, ensuring patient privacy (HIPAA), participating in quality improvement activities, and coordinating with other Center departments.

• Adherence to applicable Federal laws/regulations, OSHA, HIPAA, state and local laws, and American Medical Association guidelines.

• Compliance with standards set by the Office of the Director, Job Corps, and the Department of Labor (DOL).

• Contractor personnel must coordinate with the Health and Wellness Director and/or Center Director.

• Monthly reporting on health-related matters, including student medical separations, costs of special services, equipment needs, and supply/medication status.

• Participation in Job Corps assessment teams and DOL/Forest Service assessments.

• Contractor performance will be evaluated via the Contractor Performance Assessment Reporting System (CPARS).

• Medical services required on-site at Columbia Basin JCCC, Moses Lake, WA.

• Services are required weekly, with days and hours to be mutually agreed upon (typically 1-2 days/week, M-F, 8 am – 5 pm).

• Schedule may vary based on Center On Board Strength (OBS) and PRH changes, with modifications executed via contract modification.

• Services are not required on specified Government holidays.

• Providers must possess an active, unrestricted license to practice independently in the state.

• Ability to work independently and as part of a healthcare team.

• Experience with financially disadvantaged youth aged 16-24.

• Supervision of Nurse Practitioner/Physician Assistant may be required.

• Prompt review of applicant files and initial examinations.

• Timely and accurate medical documentation on Student Health Record (SHR) and Job Corps forms.

• Collaboration with Center staff and adherence to infection control policies.

• Contractor personnel must agree to a criminal background check and drug testing if required.

• Malpractice/professional liability and general liability insurance are required.

• Contractor personnel and their work will be secured per Center requirements.

• Student records and reports are property of the US Government.

• Travel is not anticipated or authorized.

• Invoices submitted monthly via IPP.

• Past performance evaluations will be conducted annually and at contract completion via CPARS.
