Statement of Work (SOW) / Performance Work Statement (PWS)

• Regular working hours are 0730-1600, Monday through Friday, excluding Government holidays.

• Work outside regular hours requires COR approval with at least 15 calendar days' written notice.

• Utility cutovers and interruptions require a minimum of 14 calendar days' notice.

• The Contractor shall not operate or disturb control devices in the station utilities system.

• In case of encountering Unexploded Ordnance (UXO), immediate contact with Military Police, EOD professionals, and the Contracting Officer is required. All contractor personnel performing ground disturbing activities must complete UXO awareness training.

• A Schedule of Prices must be submitted within 15 calendar days of notice of award.

• Invoices must be processed in accordance with FAR 52.232-27 and FAR 52.232-5.

• The Contractor's invoice must be on NAVFAC Form 7300/30 and include QC certification.

• Monthly Estimates for Voucher are on NAVFAC Form 4330/54.

• Monthly Work-hour reports, Construction Progress Schedules, Safety Self Evaluation Checklists, submittal registers, solid waste disposal reports, certified payrolls, and updated testing logs are required.

Statement of Work (SOW) / Performance Work Statement (PWS)

This document outlines the requirements for the repair of a culvert located on Steel Tower Road within the Verona Loop Training Area at MCB Camp Lejeune, North Carolina.

• Demolition of approximately 1000 square feet of existing 18-inch thick concrete roadway above the culvert.

• Removal of approximately 100 linear feet of existing 48-inch corrugated metal pipe from the stream bed.

• Removal of existing rip-rap as necessary to facilitate new concrete headwall construction.

• Installation of 96 linear feet of new 48-inch Reinforced Concrete Pipe (RCP) to replace the existing corrugated metal pipe.

• Installation of new concrete headwalls on the upstream and downstream ends of the new culvert pipes, in accordance with NCDOT details.

• Backfilling behind headwalls with stabilized rip-rap and geotextile, with slopes no steeper than 3:1.

• Backfilling of excavation with Aggregate Base Course (ABC), compacted in 6-inch lifts to 95% density.

• Roadway backfill to match existing concrete elevation or provide a minimum of 24 inches of cover over the new RCP, whichever is greater.

• Seeding and permanent vegetative cover (Centipede sod) for all disturbed areas upon completion of grading and construction activities.

• All work must comply with applicable Federal, State, Local, and Base regulations.

• Workmanship must be of high quality, meeting industry-approved methods.

• Final inverts of the new pipe must be 1 foot below the stream bed elevation.

• All pipe joints shall be sealed, taped, and wrapped in a layer of silt fence.
