This document is Appendix D, a Past Performance Assessment questionnaire. It is used by the U.S. Army Contracting Command to gather information from entities who have contracted with offerors for a Request for Proposal (RFP) Number W56ZTN-25-R-0001. The purpose is to evaluate offerors' past performance on recent and relevant government or commercial contracts.

• Rating Definitions: Provides definitions for "Substantial Confidence," "Satisfactory Confidence," "Limited Confidence," and "No Confidence" to standardize evaluations.
• Part I: To be completed by the Offeror: Collects identification details of the contract being assessed (Contractor Name, Address, Contract Number, Type, Award Date, Completion Date, Contract Value, etc.) and the offeror's representative.
• Part II: Evaluation (To be completed by Point of Contact – Respondent): This section requires the respondent to rate the offeror's performance in several areas, including:
  • Compliance of Products, Services, Documents, and Related Deliverables to Specification Requirements and Standards of Good Workmanship
  • Effectiveness of Project Management (including use and control of subcontractors)
  • Timeliness of Performance for Services and Product Deliverables
  • Effectiveness in Forecasting and Controlling Estimated Costs (for Cost Reimbursement Type Contracts Only)
  • Commitment to Customer Satisfaction and Business-like Concern for its Customers' Interest
  • General Comments
  • Other Information Sources (awareness of other past efforts)
• Part III: Return Information: Instructions for returning the completed questionnaire.

This questionnaire is sent to entities that have previously worked with offerors. The responses will be used by the government to assess the past performance of offerors, which is a critical factor in the source selection process for RFP Number W56ZTN-25-R-0001. Bidders should ensure that their past performance is accurately represented and that their points of contact are aware of this request.

Solicitation / RFP / RFQ

This solicitation is for a Multiple Award Task Order Contract (MATOC) for post-wide construction services at Aberdeen Proving Ground, Maryland. The contract will be a firm-fixed-price, indefinite delivery, indefinite quantity (IDIQ) type, with a five-year base period and a potential six-month extension. The NAICS code is 236220 with a size standard of $45.0M, and this action is an 8(a) Small Business set-aside.

The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other non-personal services necessary to perform post-wide construction. This includes, but is not limited to, real property maintenance, alteration, renovation, new construction, building demolition and repair, base utility maintenance and repair, and environmental-related services. All work must be in accordance with the Performance Work Statement (PWS), specifications, and terms and conditions included in the solicitation.

• Contract Type: Firm-Fixed-Price, Indefinite Delivery/Indefinite Quantity (IDIQ)

• Period of Performance: Five (5) year base contract with a six (6) month extension option.

• Minimum Guaranteed Amount: $10,000.00 per MATOC contractor.

• Anticipated Not-to-Exceed Ceiling: Approximately $121.3M for the MATOC, including base and option periods.

• Payment Terms: Partial Payment - Net 14 days; Final Payment - Net 30 days.

• Proposal Submission Deadline: 17 February 2026, 10:00 AM local time.

• Proposal Validity: 90 calendar days from receipt of proposal.

• Questions Deadline: 28 January 2026, 10:00 AM.

• Submission Method: Electronically via the DOD SAFE site.

• Proposal Structure: Four volumes (Technical, Past Performance, Price, and Solicitation, Offer, Award Documents with Certifications).

• Evaluation Factors: Technical (most important), Past Performance, and Price. Technical and Past Performance combined are significantly more important than Price.

• Evaluation Approach: Best value trade-off. Offerors must be rated at least acceptable for all factors and subfactors.

• This action is a competitive 8(a) set-aside.

• Offerors must be registered in the System for Award Management (SAM) under NAICS 236220 (Size Standard $45.0M) prior to contract award.

• Performance and payment bonds will be required at the task order level when necessary.

• General wage decision numbers are applicable and referenced in the solicitation.

• The solicitation includes numerous FAR and DFARS clauses incorporated by reference and in full text.
