Scope of Work

The new hospital will serve a projected user population of 19,088 by 2030 and will expand services to include Primary Care, Telemedicine, Dental, Eye, Audiology, Emergency, Transitional Care, various Specialty Cares (e.g., General Surgery, Orthopedics, Cardiology), Psychiatry, Social Services, Behavioral Health, Acute Care, Low-Risk Labor & Delivery, Pharmacy, Laboratory, Diagnostic Imaging, Rehabilitation, Public Health, Wellness Center, and more. The project also includes the construction of two wastewater treatment plants (WWTPs), improvements to AZ State Highway 73, upgrades to the Tribal Utility’s surface water treatment plant (SWTP), and replacement of a section of sewer main. The existing hospital will be demolished after the new facility is built and services are transitioned.

Evaluation & Submission
This is a Request for Proposals (RFP) utilizing a Best Value-Tradeoff evaluation. Criteria include Technical Experience, Technical Approach/Schedule/Logistics Plan, Key Personnel/Organizational Chart, Past Performance, and Price. Proposals must be submitted electronically in PDF format. Required forms include the Key Personnel Resume Form (J08), Company Technical Experience Construction Sheet (J07), and Past Performance Questionnaire (J09). Large businesses must also submit a Subcontracting Plan (J10).
