Statement of Work (SOW)

The objective of this contract is to remove approximately 2000 square feet of existing resilient tile squares and carpet tiles and replace it with commercial-grade Luxury Vinyl Plank (LVP) 20mil flooring, including the installation of a 6" cove base and thresholds.

• Pre-Work Preparation: Conduct site inspection, finalize LVP color/design and cove base selections, submit material submittals for flooring and adhesive, move furniture and modular desk sections, and protect non-work areas from dust and debris.

• Removal of Existing Flooring: Remove approximately 2000 sq ft of existing resilient tile and carpet tiles, dispose of removed materials according to regulations, and use abatement methods to protect non-work areas.

• Subfloor Preparation: Inspect subfloor for damage, report issues, and prepare the subfloor to ensure a smooth and level surface (cleaning, patching, sanding).

• Installation of Commercial LVP Flooring: Install 25mm commercial LVP flooring per manufacturer specifications, ensuring alignment and consistent appearance. Flooring should flow seamlessly between hallway and breakroom areas. Use manufacturer-recommended adhesive.

• Installation of 6" Cove Base and Thresholds: Install 6" cove base around the perimeter and install transitional thresholds for 7 single doors and 1 double door entry. Specific transitions include LVP to travertine tile (3 doors) and LVP to concrete or tile (5 doors).

• Post-Work Clean-Up: Clean new flooring, remove adhesive residue/debris, return furniture, dispose of project waste, and ensure the work area is clean and orderly.

• Location: VBA Boise Regional Office, 444 West Fort Street, Boise, ID 83702.

• Duration: The Regional Office is expected to close for no longer than 5 weekdays. Work may be performed on weekends. Project must be completed within 7-9 days (5 weekdays and up to two weekends).

• Commencement: Installation shall commence within 45 days of contract award.

• Security: Contractor must identify at least 2 personnel for fingerprinting and PIV-I badge issuance at least 10 days prior to work commencement. One PIV-I badge holder must be on-site at all times. All personnel must wear visible badges. Contractor personnel and vehicles are subject to search by VA police. Weapons are prohibited. No photography of VA premises without written permission. VA reserves the right to shut down the project in case of a national emergency.

• Deliverables: Material submittals, documented pre-work site inspection report, and final inspection report.

• Quality Assurance: Contractor must implement a quality control plan. Final installation will be inspected and approved by the COR/facility manager.

Amendment / Modification

This amendment modifies Solicitation 36C10D26Q0077 with the following key changes:

• Extended Offer Receipt Date: The deadline for submitting offers has been extended to Tuesday, April 28, 2026, at 1:00 PM Mountain Time (MT).

• Rescheduled Site Visit: The site visit is now scheduled for Tuesday, April 21, 2026, at 10:00 AM MT.

• Updated Question Deadline: The deadline for submitting questions regarding the RFQ or site visit is now Thursday, April 23, 2026, at 1:00 PM MT.

• Amended Past Performance Questionnaire Due Date: The due date for Past Performance Questionnaires is now Tuesday, April 28, 2026, by 1:00 PM MT.

• SOW Installation Commencement: Paragraph 3.3 of the Statement of Work (SOW) has been amended to change the installation commencement period from "within 30 days of contract award" to "within 45 days of contract award."
