Statement of Work (SOW)

The objective of this contract is to remove approximately 2,238 square feet of existing resilient tile squares and carpet tiles, and replace it with commercial-grade Luxury Vinyl Plank (LVP) 20mil flooring, including the installation of a 6" cove base and thresholds at the VBA Boise Regional Office.

• Pre-Work Preparation: Conduct site inspection, confirm square footage, finalize LVP color/design selections (samples required for COR approval), submit material submittals for flooring and adhesive, move furniture and modular desk sections, and implement dust/debris protection measures.

• Removal of Existing Flooring: Remove approximately 2,238 sq ft of resilient tile and carpet tiles. Dispose of removed materials in compliance with regulations. Protect non-work areas.

• Subfloor Preparation: Inspect subfloor for damage, report issues, and prepare the surface (cleaning, patching, sanding) for new flooring.

• Installation of Commercial LVP Flooring: Install 20mm commercial LVP flooring according to manufacturer specifications, ensuring proper alignment and seamless flow between areas. Use manufacturer-recommended adhesive.

• Installation of 6" Cove Base and Thresholds: Install 6" cove base around the perimeter and transitional thresholds for various door entries (LVP to carpet, LVP to travertine, LVP to concrete/tile).

• Post-Work Clean-Up: Clean installed flooring, remove adhesive residue, return furniture, dispose of waste, and leave the work area clean.

• Location: VBA Boise Regional Office, 444 West Fort Street, Boise, ID 83702.

• Duration: The office is expected to close for no longer than 5 weekdays. Work may be performed on weekends. Project must be completed within 7-9 days (5 weekdays and up to two weekends).

• Commencement: Installation shall commence within 45 days of contract award.

• Security: Contractor must identify at least 2 personnel for fingerprinting and PIV-I badge issuance at least 10 days prior to work commencement. One designated person must be on-site at all times. All personnel require visible badges. Contractor personnel and vehicles are subject to search. No photography of VA premises without written permission. Weapons are prohibited.

• Deliverables: Material submittals, documented pre-work site inspection, and final inspection report.

• Quality Assurance: Contractor must implement a quality control plan. Final installation subject to COR/facility manager approval.

Amendment / Modification

This amendment modifies the solicitation for flooring replacement:

• Clarification of SOW: Corrects a typographical error in paragraph 2.4 of the SOW, changing the specified flooring thickness from "25mm" to "20mm".

• Pre-Work Preparation: Conduct site inspection, finalize LVP color/design and cove base selections, submit material submittals for flooring and adhesive, move furniture and modular desk sections, and protect non-work areas from dust and debris.

• Removal of Existing Flooring: Remove approximately 2000 sq ft of existing resilient tile and carpet tiles, dispose of removed materials according to regulations, and use abatement methods to protect non-work areas.

• Subfloor Preparation: Inspect subfloor for damage, report issues, and prepare the subfloor to ensure a smooth and level surface (cleaning, patching, sanding).

• Installation of Commercial LVP Flooring: Install 25mm commercial LVP flooring per manufacturer specifications, ensuring alignment and consistent appearance. Flooring should flow seamlessly between hallway and breakroom areas. Use manufacturer-recommended adhesive.

• Installation of 6" Cove Base and Thresholds: Install 6" cove base around the perimeter and install transitional thresholds for 7 single doors and 1 double door entry. Specific transitions include LVP to travertine tile (3 doors) and LVP to concrete or tile (5 doors).

• Post-Work Clean-Up: Clean new flooring, remove adhesive residue/debris, return furniture, dispose of project waste, and ensure the work area is clean and orderly.

• Location: VBA Boise Regional Office, 444 West Fort Street, Boise, ID 83702.

• Duration: The Regional Office is expected to close for no longer than 5 weekdays. Work may be performed on weekends. Project must be completed within 7-9 days (5 weekdays and up to two weekends).

• Commencement: Installation shall commence within 45 days of contract award.

• Security: Contractor must identify at least 2 personnel for fingerprinting and PIV-I badge issuance at least 10 days prior to work commencement. One PIV-I badge holder must be on-site at all times. All personnel must wear visible badges. Contractor personnel and vehicles are subject to search by VA police. Weapons are prohibited. No photography of VA premises without written permission. VA reserves the right to shut down the project in case of a national emergency.

• Deliverables: Material submittals, documented pre-work site inspection report, and final inspection report.

• Quality Assurance: Contractor must implement a quality control plan. Final installation will be inspected and approved by the COR/facility manager.

Amendment / Modification

This amendment modifies Solicitation 36C10D26Q0077 with the following key changes:
